It aims to be very practical and logical, providing a clear summary of the evidence, and a strategic pathway through the minefield of what we do when things go wrong in healthcare. It also aims to be emotive, because ultimately we all suffer emotionally when things go wrong: to avoid the emotion is to court disaster. This chapter arises from the author’s direct involvement in the development and delivery of the 2003 Australian Open Disclosure Standard and the rollout of open disclosure training across NSW Health. It is rooted in the actual experiences of more than a thousand senior clinicians and health administrators. |
